Filling Out the Intake Form
To kickstart the process, please fill out the Intake Form for a free intake session. This form is vital for us to learn about your concerns and gather essential details about your child’s medical and neurodevelopmental background. You can download the Developmental History Form or opt to receive it via email. Additionally, please provide copies of any previous assessments. This depth of information is key to crafting an effective evaluation and support plan.

Assessment and Feedback
Our thorough assessment includes standardized testing, clinical observations, and parent-report measures, offering insights to inform our treatment design. Following this, we’ll invite you to a Feedback Session—without your child—to discuss the findings, the assessment results, and outline a potential treatment strategy.
Step 3
Customized Therapy Plan
Therapy is personalized, reflecting each client’s developmental stage, diagnosis, and family priorities. For those benefiting from an intensive approach, we offer a 30-session program, incorporating occupational and listening therapies, supplemented by regular parent meetings for ongoing support and education. Post-therapy evaluations ensure progress tracking, with options for “booster” sessions to address new developmental milestones as they arise.

Our son has recently started first grade. With your help, in the space of one school year our son has gone from needing a full time Para-Educator in the classroom to being completely independent with no need for inclusive support. Our son could not have accomplished and overcome half of what he has without your dedication and support. What a blessing this has been to our son and our family.
I just want you to know how INCREDIBLE our Occupational Therapist has been to work with. Our child does not warm up to people easily, but he has been able to connect with her better than any other provider we have ever had! He loves his sessions with his OT and she has helped him make tremendous progress.
